
Opatz and Struzyk purchased their first care center in May of 2012 from Paynesville Area Health Care Center. Today they own and operate many independent living, assisted living, memory care and skilled care facilities. Prior to the start of Premier Health Care Management and later on CURA, Opatz worked for a large not for profit senior care organizations in a variety of roles including Vice President of operations and Director of Real Estate development. During his tenure, he developed and constructed nearly 200 million in development in senior living. He has extensive experience with financing projects to include bonds, bank placement and HUD. He is a Licensed Health Services Executive with a bachelor’s degree in public administration with an emphasis in health care administration from St Cloud State University.
Fred Struzyk is a Licensed Health Services Executive. Fred served as an administrator of health care facilities for the past 39 years. Hospitals, Nursing Homes, Clinics and Assisted Living facilities located in Central Minnesota. These facilities have ranged from independent to nationally chained based organizations. His passion has been Long Term Care. Areas of expertise is nursing home and assisted living reimbursement and daily operations. Fred is CFO of CURA and Co-Owner of and Premier Healthcare Management. Fred earned his degree in Business Management at Concordia University, St. Paul.

David Johnson is an attorney admitted to practice in Minnesota in May of 1999. He earned his law degree from Hamline University School of Law in St. Paul in December of 1998 and graduated from Gustavus Adolphus College in St. Peter in May of 1988. His areas of practice include real estate, family, business and commercial law, collections, estate planning, wills and trusts, elder law (including guardianships, conservatorship, and Medical Assistance planning), litigation and appeals, probate, and estate administration. David is a member of the Minnesota State Bar Association and is active in the Paynesville community through the Chamber of Commerce, Dollars for Scholars, the Paynesville Area School District Board, and PayDelCo.

Scott Bernatz has been a healthcare leader for the last 16 years. He most recently served as Regional Sales Manager for ZOLL Medical’s Cardiac Management Solutions Division for Minnesota, North Dakota, and Wisconsin (2022–2025), supporting outpatient solutions for heart failure, sudden cardiac arrest risk, and at-home sleep apnea diagnostics. From 2012–2022, he was Heartland Regional Sales and Operations Manager with Sizewise (now Agiliti Health), helping deliver specialty bed frames, mattresses, and patient mobility and lift equipment across long-term and hospital settings with a focus on wound prevention. Scott lives in Farmington with his wife, Holly, and their two children and has volunteered in local baseball and theatre programs.

Shelly Cornish is an executive healthcare leader and Licensed Nursing Home Administrator with over 20 years of experience advancing operational excellence, strategic planning, and person-centered care in senior living and long-term care. She has served as Regional Operations Director and Executive Director with Cassia / Castle Peak Senior Life & Rehabilitation in Colorado since 2019, overseeing skilled nursing, memory care, and assisted living communities with a strong focus on regulatory compliance, quality outcomes, occupancy, and financial performance. Prior to this, Shelly held multiple leadership roles with Ecumen from 2004–2019, including executive oversight of a large, multi-service campus and leading teams, budgets, and capital projects while maintaining strong CMS performance. She earned a Master of Science in Gerontology and a Bachelor of Science in Social Work from Minnesota State University, Mankato, and has contributed to aging and CNA advisory committees.
